If you are an adult making an appointment for yourself, please download and complete the Consent to Treatment, Payment Policies, and Privacy Form.
If you are an adult making an appointment for a child, please also download and complete the Consent for Child Treatment form, which must be signed by both parents, and as the Payment Policies, and Privacy Practices form.
