If you are an adult making an appointment for yourself, please download and complete the Consent-For-Treatment forms.
If you are an adult making an appointment for a child, please also download and complete the permission for Child Treatment form, which must be signed by both parents, and fill out the Child Developmental Questionnaire as well as the Intake and Consent-For-Treatment forms.
Please bring forms to your first appointment.
